Circular(58): Mandatory Information For Patient Registration Forms

Reference: CIR-2016-SHN0640

Kindly note that the Health Regulation Department mandates all healthcare facilities in the Emirate of Dubai to incorporate the following mandatory information through their registration form (electronically or manually) in addition to the existing registration information:

o Proof of Identity

o Nationality

o Country of Residence

o Specify Emirate of Residency for Residents in UAE
